What Is PACE? Helping Indiana Seniors Age with Independence and Dignity


As Indiana's population continues to age, more families are searching for ways to help their loved ones remain safely at home while receiving the healthcare and support they need.


One solution making a meaningful impact across the state is the Program of All-Inclusive Care for the Elderly (PACE).


PACE is a comprehensive care program designed for individuals age 55 and older who qualify for a nursing facility level of care but can continue living safely in their homes and communities with the right support.


Who Qualifies for PACE?

To be eligible for PACE in Indiana, individuals must:

  • Be 55 years of age or older

  • Meet the state's nursing facility level of care requirements

  • Be able to live safely in the community at the time of enrollment

  • Reside within a designated PACE service area


The goal of PACE is simple: provide coordinated, person-centered care that helps participants maintain their independence and quality of life for as long as possible.


Comprehensive Care Through One Coordinated Program

PACE brings together medical care, supportive services, and social engagement through a single care team. Rather than navigating multiple providers and programs, participants receive coordinated care tailored to their unique needs.


Services may include:

  • Primary and specialty medical care

  • Hospital and emergency services

  • Prescription medications

  • Home care services

  • Nursing home care when needed

  • Physical, occupational, and recreational therapy

  • Adult day services

  • Transportation

  • Dental care

  • Nutritional counseling

  • Social services and counseling

  • Laboratory and diagnostic services

  • Meals and wellness support


By coordinating these services through one program, PACE helps reduce gaps in care while providing peace of mind for participants and their families.


How the PACE Model Works

PACE operates within Indiana's managed care system. Each participant is enrolled with a PACE organization that coordinates and manages all covered services.


Because care is coordinated through a single organization, participants agree to receive services from providers, hospitals, pharmacies, and specialists that are part of the PACE network. This integrated approach allows the care team to oversee all aspects of a participant's health and well-being, helping ensure services are delivered efficiently and effectively.


Supporting Independence Across Indiana

PACE is more than a healthcare program. It is a model of care that empowers older adults to remain connected to their families, homes, and communities while receiving the support they need.


For many participants, PACE provides an alternative to long-term institutional care, allowing them to age with greater independence, dignity, and quality of life.
